Am I eligible to become an AICHC patient?

Yes, all individuals are eligible to become AICHC patient.
We accept all major insurance types including Medi-Cal and Medi-Care.

In addition, we also offer "Sliding Fee Scale", and there are some requrements to meet if uninsured

  • you do not have health insurance of any kind
  • your income does not exceed 200% of the Federal Poverty Level Guidelines

How do I register to become a patient at AICHC?

In order to register, you must come to a AICHC clinic during our intake/registration hours.

Please bring the following information with you at the time of intake:

  • Picture ID
  • List of current medications
  • Copy of all past medical records (if you have seen a physician in the last three years)
  • Letter of termination from insurance company (if applicable)
  • * Proof of income may be required for certain programs

Do I need an appointment to see a doctor or a nurse ?

No, at AICHC we welcome walk-in patients. However, to experience smooth and fast service it is advised so you have an appointment. Please call and make an appointment.

How soon will I get an appointment with a doctor after I register?

Depending on the number of new patients being scheduled, an appointment may be available immidiately or for non-urgent service it may take from 2 up to 7 days to be seen by a doctor at AICHC.

If I run out of medication, can I come in for a refill?

In order to receive a refill on your medication, you must be a registered patient, be seen by a medical professional and may need tp complete any diagnostic tests required by the practitioner. Please call and make an appointment.

Do you have evening and weekend appointments?

Yes, our clinic is open every other Saturday from 9:00 am to 2:00 pm.

If I only need a TB test, can I get it done at AICHC?

All of these tests/visits can be done at AICHC. Services onsite are only available to active AICHC patients. If you are not an active patient, you will need to register to become a patient to be seen. A provider will need to order all lab and other tests, so please contact us at 818-843-9900 for an appointment.

If I only need a physical for my new job, can I get it done at AICHC?

Yes. AICHC providers can complete physicals for all active patients. If you are not an active patient, you will need to register to become a patient to be seen.

Do I need to have a Social Security number to become a patient?

No, you do not need a Social Security number to become a patient at AICHC.

How can I get copies of my medical records?

In order for us to release your medical records, you must sign a release of authorization. Please stop by the front desk and fill one out. Once we have received the release, we will send it to you within 10 business days.

How do I access my provider after hours?

If you need to get in touch with the office after regular business hours or on the weekend, we do have an answering service that you are welcome to contact.  It may take several minutes to get through the automated system and reach an operator, so please be patient.  The answering service will not take messages for prescription refills.
